SCHOOLS, DISTRICTS & Organizations please use this form to provide needed information for any number of member registrations.

1. We need you to provide us with your contact information along with information for each teacher who is included in your group registration
2. The District Representative page is for the contact person responsible for registering multiple teachers.
3. Enter each teacher’s registration information within part 2 (Member Registration). If the contact person is also becoming a member or renewing an existing membership, their registration must be added to a Member Registration page.
   
4. All Member Registration information MUST BE to be entered, using one page per teacher.
5. Submit form when completed. You will receive an email with all information as you entered it.
6. After completing the teacher information go to Annual Group Memberships Page and select the number of members you are registering, then complete the payment process for each category.
7. You will be charged depending on the number of registrations – 5 or more receive a 25% discount
   
8. You may PAY by PO, CC or PayPal
   
9. EMAIL your PO directly to  astaexecdir@azsta.org
